As the lead provider of support for people living with Dementia, their family and Carers in North Lincolnshire, we strive to ensure that North Lincolnshire is a Dementia Friendly and Carer Friendly place to live. We do this in a number of ways:

• Awareness raising with health and social care – to ensure professionals are aware of services and support, and how to refer
• Working towards a clear Dementia Pathway, so that anyone with concerns about their memory knows the steps they need to take to find the cause of their concerns, the services available to support them, and what they can expect from each service.
• Working with the community, encouraging people to become Dementia Friends and places to become Dementia friendly, so that those living with Dementia are able to live as independently as possible, with confidence.
• Ensuring those living with Dementia, their family and Carers are on the agenda when key decisions are made about services and support.
• Giving those living with Dementia, their family and Carers a voice, ensuring their views are taken into account when services and support are being designed, developed and implemented.
